Richmond County Hospice operates this Website to share information about our hospice and palliative care services, our facilities, our community events, and the ways people can support our work. Visitors may use the Website to make a patient referral, apply to volunteer, and find contact details for our offices.

The Website is informational. Nothing on it is medical advice, a diagnosis, a treatment recommendation, or an admission decision, and nothing on it creates a provider-patient relationship.

The Website is intended for a general adult audience. Referral and volunteer forms should be submitted by someone 18 or older. Volunteers under 18 are welcome to take part in our programmes, but a parent or legal guardian should complete the application on their behalf.

The Website is not directed to children under 13, and we do not knowingly collect personal information from children under 13.

We reserve the right to refuse access to the Website or its services to anyone for any reason at any time.

4.Referral and volunteer forms

When you submit a referral or a volunteer application through the Website, you agree to provide accurate and complete information.

Submitting a referral does not by itself admit anyone to our care. Admission depends on eligibility, a clinical assessment, physician orders, and bed or service availability. Our Intake Department will contact you to discuss next steps.

We will make reasonable efforts to respond promptly, but we do not guarantee a response time through the Website. If a situation is urgent, please call us at 910-997-4464 rather than using a form.

Our forms are delivered by email and are not a secure channel. Please do not submit Social Security numbers, financial account details, payment card numbers, or detailed medical records through them.

In a medical emergency, call 911.

5.Intellectual property

All content on the Website, including but not limited to text, images, graphics, logos, icons, photographs, audio clips, video clips, and software, is the property of Richmond County Hospice, Inc. or its content suppliers and is protected by applicable copyright, trademark, and other intellectual property laws.

Donations and ticket purchases are completed on those providers' own websites, under their terms.

9.Service availability and Website downtime

We make commercially reasonable efforts to keep the Website accessible and operational. However, we do not guarantee that the Website will be available at all times or free from interruptions, errors, or technical issues.
