The Website provides a referral form, used to tell us about someone who may benefit from hospice care, and a volunteer application form. Both send the information you enter to our staff by email.
We use referral information to contact you, to understand the person's situation, and to pass the referral to our Intake Department. We use volunteer application information to assess and follow up on your application, and to maintain volunteer records.
If you are referring another person, please share only what is needed for us to make contact. Our Intake staff will gather any further detail directly and through the appropriate channels.
4.Health information and HIPAA
Richmond County Hospice, Inc. is a healthcare provider. Information about a patient's health, care, or treatment that we hold in that capacity is protected health information and is governed by the Health Insurance Portability and Accountability Act (HIPAA) and by our Notice of Privacy Practices.

Even after you opt out of newsletters, we may still contact you about a referral, a volunteer application, an event you registered for, or a matter concerning care.
